Syndromes of Traditional Chinese Medicine. ✵Insomnia and Inability to sleep syndrome:Insomnia is a syndrome of not being able to fall asleep easily and naturally, and is the subjective experience of one who is not satisfied with the duration and quality of his or her sleep, and it affects daytime social functioning.

Inability to sleep (insomnia, sleeplessness).

 Inability to sleep is a syndrome of not being able to fall asleep easily and naturally, and is the subjective experience of one who is not satisfied with the duration and quality of his or her sleep, and it affects daytime social functioning. Inability to sleep (bù mèi) is also known as insomnia, sleeplessness, bù dé wò (imcapability of sleeping).

 The common symptoms of inability to sleep (insomnia, sleeplessness) include difficulty in falling asleep, decreased sleep quality, loss of memory and concentration. Inability to sleep (insomnia, sleeplessness) may be short-term, lasting a few days to a week, or long-term, lasting more than a month.

 Main Causes: insomnia is normally caused by mental stress, psychosocial factors, certain chronic diseases, etc.

 Main Symptoms: symptoms of insomnia are difficulty in falling asleep, poor quality of sleep, easy to wake up, and one also has forgetfulness, sleepiness in daytime.

 Diagnostic criteria for insomnia in adults:

 ①.Insomnia presents as difficulty in falling asleep, one can fall asleep more than 30 minutes;

 ②.Sleep quality declines, disorders of sleep maintenance, the number of awakenings throughout the night is over twice, early awakening, sleep quality declines;

 ③.The total sleep time is reduced, usually less than 6 hours.

 The above symptoms are accompanied by concomitant daytime dysfunction. Sleep-related daytime functional impairments include:

 ①.Fatigue or generalized uncomfortableness;

 ②.Decreased concentration, hypoprosexia (aprosexia or loss of attention), or hypomnesis (failure of memory);

 ③.Decreased ability in study, work and social competence;

 ④.Anxious state of mind or irritability;

 ⑤.Daytime sleepiness (drowsy in daytime);

 ⑥.Decreased interest and energy;

 ⑦.Increased tendency to make mistakes during work;

 ⑧.Tension, cephalalgia (headaches), dizziness, or other physical symptoms related to sleep loss;

 ⑨.Show overmuch attention to sleep.

 Etiology of Insomnia:

 The ancient classical works Ling Shu (the Spiritual Pivot or Divine Axis) states:"The old man's Qi and blood declines,...so one is not spiritual in daytime and sleepless at night".

 In ancient TCM books, insomnia or sleeplessness is known as inability to sleep (bù mèi), or bù dé wò (imcapability of sleeping). The main syndromes of inability to sleep (insomnia, sleeplessness) are introduced:                      

 (1).Sleeplessness due to disorder of the stomach and dyspepsia: on has discomfort in the stomach and has uneasiness in sleeping, distension, aching and pain in the stomach, it is a syndrome of dyspepsia (indigestion).                      

 The stomach Qi flows downward, if the stomach Qi is upward, it is congested in the lungs and there is a sound in the breath, and it is not possible to flow down, and there is sleeplessness.                      

 (2).Blood deficiency of the heart: those who have blood deficiency of the heart, and restlessness in sleeping, due to thinking too much.                      

 One is sleepless, deficiency of the heart blood, there is a febrile disease. However, although the channel of the disease belongs to the heart, in fact, all the five Zang-viscera are involved. For the heart blood is insufficient, or mind is not able to keep its place, thus leads to insomnia.                      

 (3).Liver deficiency: there is a deficiency of the liver and the pathogenic factors affect the liver, then could not sleep, the liver is hurt and one is easily getting angry, one cannot sleep further.                      

 (4).Wind, cold, and pathogenic heat invading the heart: there are wind, cold and pathogenic heat affecting the heart, or summer heat affect the heart, resulting in restlessness and uneasiness.                      

 (5).Cold pathogen disturbing the mind: there is pathogenic cold in the interior, one is restless and uneasy.                      

 (6).Panic and restlessness: there is panic and restlessness, one is frightened and fear in the dream.                      

 (7).Phlegm and dampness: there are dampness and phlegm oppress the heart and leads to uneasiness, vomiting, nausea, oppression in chest and diaphragm.                      

 (8).Insufficiency of Yin and Yang is floating: the Yin is insufficient and hurt, the Yang is floating, fire is motivated, Qi is uneasy, thus one is sleepless.                      

 (9).The bedding is too thick: bedclothes is too warm and cold, and seasonal climate is uneven of hot and cold, all making it impossible to sleep peacefully, it is not related to the disease, should be careful and not treat it wrongly.

 There are inability to sleep (insomnia, sleeplessness) from miscellaneous diseases. In a summary, the cause of inability to sleep (insomnia, sleeplessness), also due to miscellaneous diseases, and the transfer of pathogens, these syndromes of insomnia are introduced:                      

 (1).Laboring with the mind: those persons who are busying with their mind, often have sleeplessness.                      

 (2).Those who using their mind much and there is pathogenic cold in gallbladder, could not sleep at night.                      

 (3).Those who thinks much, causing the spleen to suffers from pathogens, pulse is slow in both hands, and one suffers from sleeplessness for many years.                      

 (4).Sleeplessness of much phlegm: one who has a phlegm syndrome, much phlegm, mostly have sleeplessness.                      

 (5).Those who are old age and mostly are sleepless.                      

 (6).Those with excessive phlegm are often sleepless.                      

 (7).Those who have dysphoria (deficient and vexed) are often sleepless.                      

 (8).There are also those who do not sleep through the night.                      

 (9).Those who wake up in a panic when they sleep.                      

 (10).There are those who are unable to sleep due to wheezing.                      

 (11).Those who cannot sleep due to consumptive disease and dysphoria with smothery sensation.                      

 (12).There are those who are sleepless because of palpitation due to fright from liver deficiency.                      

 (13).There are those who do not sleep due to dysphoria after a serious illness.                      

 (14).Those who sleep loudly and soundly while lying down, and wake up in a short while due to fire in the heart and lungs.                      

 (15).Those who do not sleep due to mania, excessive fire and phlegm accumulation.                      

 (16).Those who have a cold-induced disease, emetic method and purgation are induced, dysphoria and sleepless.                      

 (17).Those who are deficient in heart and gallbladder, easily frightened by things, have many ominous dreams, dysphoria and sleepless.                      

 (18).Those who have lost their will and depression, stagnation of phlegm and salivation in the heart, severe palpitation and sleepless.                      

 (19).There is a restlessness of the spirit and Qi, and every time one lies down, one feels that the body is in the bed and the soul is away from the body, and there are many nightmares and palpitations, and those who do not sleep through the night, this is known as the syndrome separation of the soul.
